TERMS & CONDITIONS AGREEMENT:
This Agreement is made by and between Luminocity, LLC., d.b.a "WichitaVIPCard.com" and your participating business, hereinafter referred to as the "Vendor".

The "Vendor" agrees to accept and honor the Wichita VIP Card, by extending the discount/offer written above in this Agreement for all goods and services the "Vendor" may provide its customers (Excluding any alcoholic beverages). The term of this Agreement will be for a period of one year from the date of acceptance. Such discounts will occur at the time of payment by the customer to the "Vendor". The customer will show the card to the "Vendor" or "Vendor’s" representatives for proof of card ownership. If payment is being made via physical delivery or through electronic communications, the customer may fax a copy of the card face with the account number and expiration date to the "Vendor".

The "Vendor" will receive an active web link, an advertising page with several photos, and exposure to thousands of monthly visitors to the WichitaVIPCard.com site, for the duration of this Agreement.

This Agreement cannot be changed or modified orally, and will automatically renew for an additional period of one year unless the "Vendor" elects to unilaterally cancel renewal by written notice at least thirty days prior to expiration. Such notice can be directed to WichitaVIPCard.com, PO Box 780301, Wichita, KS 67278.

I have the authority to authorize WichitaVIPCard.com to use any information needed to create and post the said website. I agree with all terms and conditions stated to me and I confirm that all information given is correct. I permit WichitaVIPCard.com to use any information regarding the company I represent in any manner neccessary in the production and listing of the site. This includes but is not limited to pictures, logos, artwork, and any other form of advertisement. I understand that WichitaVIPCard.com takes no responsibility if the information given is incorrect, owned or trademarked by any other company.
